Care Managers:
- Conduct a comprehensive needs assessment that focuses on the goals and preferences of each older adult
- Develop and implement a customized care plan, arranging services such as personal care, home-delivered meals, and transportation
- Connect individuals to needed resources
- Monitor progress through visits and phone calls and adjust care plans as needed
